In Home Support Services

Home based services are provided to frail older persons, eligible Veterans and people with a disability and their carers. These services help our clients to be active and independent in their own home. Council's HACC program offers the following services:

Home Care

Assistance with domestic tasks to maintain a safe environment.

Respite Care

Respite offers a break for a carer of a person who has a disability and/or illness.

Personal Care

Assistance with personal activities of daily living.

Delivered Meals

A nutritious balanced meal is delivered to those who are nutritionally at risk.

Home Safety Program

The Home Safety Program offers assistance with maintaining tasks around the home to ensure a safer environment.

Home Care Packages

The SupportFirst Home Care Packages program provides services and care management to assist people with complex care needs to remain living in their own home.

To register on the waiting list to receive a home care package, the person must first be assessed by an Aged Care Assessment Service to determine whether a Home Care Package is suitable. The assessment is free and can be arranged by the individual, a family member or doctor.

Each Home Care Package is tailored to suit the individual’s needs. Packages may include assistance with:

• Bathing and showering
• Home maintenance/gardening
• Social activities/transport
• Housework/shopping
• In home or community respite.

For more general information regarding home care packages including possible fees visit the My Aged Care website. For more information regarding SupportFirst contact the Intake & Assessment Officer on 5471 1766.
