The role

An exciting new role within the dynamic Community Wellbeing unit, the Home Care Package Coordinator is responsible for coordinating the Home Care Package program, in accordance with the Aged Care Quality Standards.

Key responsibilities include:

  • Provide efficient and responsive support and customer service to clients, their representatives, and other service providers
  • Provide leadership and build capacity of the Care Planners for the best outcomes for the clients
  • Support clients to participate in the design, development and review of their care plan
  • Ensure financial integrity in coordinating the Home Care Package program

 

Our ideal candidate

To be successful in this role you will have:

  • relevant experience and expertise in a similar role, and/or qualifications in the clinical or allied health fields essential
  • a good understanding of the Aged Care Sector services
  • Proficiency in the use of computer programs and My Aged Care
  • Understanding of and commitment to working within the Aged Care Quality Standards
